Patient Forms

Please download and print the following THREE  forms. Complete all THREE of the forms prior to your visit and bring them to your appointment with your Photo Identification (driver's license), Insurance Cards, Referral (if necessary) and copies of any and all lab results or test results pertinent to your visit or performed within the last six months.  You may also have your primary doctor fax your records to our office in advance of your appointment at 201-578-1699.

Laxative preparations for procedures are selected by the physician in accordance with your health parameters. Most laxative preparations require a written prescription. Below are the written directions for each of our common preparations for colonoscopy (lower endoscopy) and gastroscopy (upper endoscopy) in the event that the one given to you at the time of scheduling is misplaced or changed.

DIRECTIONS FOR COLONOSCOPY:  Since Every patient is unique, each  preparation is customized to meet the needs, age and health status of each patient.  For this reason,  we ask that you contact our scheduling department for specific instructions regarding your laxative preparation prior to procedure.

Insurance and Billing

Please contact our office to verify acceptance of your insurance plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We are happy to file insurance for your reimbursement as long as you are free to choose your own gastroenterologist.

Office visit or consultation please report to Suite 4C

Parking Instructions for all patients:
Designated parking, marked reserved for GI Associates ONLY is available immediately on the right as you enter the parking lot from Katherine Street. You may also park in any of the areas labeled Visitor, which are available around the perimeter of the parking lot. Additional spaces are also available on the lower level. These spaces are #35, #36, #37, #38 and are marked GI. Please do not park in a space that has a company name other than GI or Visitor; you will be at risk for being towed!

From Route 4 East or West
Exit Route 4 at the Teaneck Road Bergenfield Exit (This is important, since the Bergenfield Exit puts you north on Teaneck road). At the end of the ramp go straight. At the first light, make a left onto Selvage Avenue (The Teaneck Nursing Home will be on your left and St. Anasthasia church will be on your right). Make the first left onto Katherine Street and proceed into the parking area.

From Route 80 East
Take Route 80 to Exit 70 marked Teaneck/Leonia. This is just past the NJ  Turnpike South Exit. Stay in the right lane. Go to the second turnoff  marked 70B Teaneck follow the road to the second traffic light (make sure you stay towards the right as you approach the second light in order to enter the turn lane) and make a right onto Teaneck  Road.  Continue  heading north on Teaneck Road to the 5th traffic light (as soon as you go under the route 4 overpass, you will approach this light). Make a left onto Selvage Avenue (The Teaneck Nursing Home will be on your left and St. Anasthasia church will be on your right). Make the first left onto Katherine Street and proceed directly into our parking lot.

From Route 17 North and South
Take Route 17 to Route 4 East and follow the Route 4 directions listed above.

From the Garden State Parkway – Northbound
Travel the Garden State Parkway to Exit 161, which puts you on Route 4 East. Please follow the Route 4 directions listed above.
